Your first visit is all about getting to know YOU, so we spend a lot of the time digging into details. Your first visit will consist of a comprehensive musculoskeletal examination followed by an evaluation of your movement, posture, flexibility, joint mobility and strength. We will discuss the root cause of your injury, prognosis and game plan moving forward based on your goals. We will always include treatment and education for self-management on the first day to get you on the right track.
We will reassess your progress at each follow-up visit and make modifications to your plan of care to ensure we are on the right path to get you where you want to be!

We are an out-of-network provider as we do not contract directly with insurance companies. This gives us the freedom to focus time and energy on high-quality care with no limitations on time or treatment provided. We can provide you with a “super bill” to submit a claim to your insurance provider for possible reimbursement. Click here to learn more about the benefits of cash-based physical therapy!

At your request, we can provide you with a “superbill” which is a document that contains the demographic information and codes your insurance company will need to submit your claim. If approved, your payments to Resurgence can apply towards your deductible and you may receive reimbursement if you choose to submit your bill using your out-of-network benefits. However, please know that this is no guarantee and it is your responsibility to submit to your insurance if you choose to do so.

You have the right to a “Good Faith Estimate” explaining how much your services will cost. This new law requires providers to give patients who do not have insurance or who are not using their insurance, an estimate of the cost for services provided. You have the right to receive a Good Faith Estimate 1 day before treatment and may request an estimate prior to scheduling your appointment.
